The most important aftercare tips are simple: leave the treated area alone for the first 24 hours, skip strenuous exercise and heat, use cold compresses for swelling, sleep with your head slightly elevated, and drink plenty of water. Follow these basics and most swelling and bruising will fade within a week, leaving you with the smooth, balanced result you came in for.
At VitaGrace Aesthetics in Rockwall, TX, dermal filler treatments are performed under medical supervision, and aftercare is part of the plan from the moment you sit in the chair. Below is a practical, day-by-day guide to what happens after your appointment and how to protect your investment while your results settle.
Filler results are visible right away – that’s one of the reasons people love it. Whether you’ve had cheek enhancement, chin filler, lip augmentation, or product placed to soften the pyriform area, you’ll notice added volume the second you look in the mirror. What you see on day one is not your final result.
The treated area may feel firm, tender, or tight – especially with lip filler. Most hyaluronic acid–based fillers attract water, so part of what you see initially is temporary swelling rather than product volume. Small bumps or unevenness under the skin are common and usually smooth out over the first week or two.
Typical short-term effects after filler include:
Swelling – the body’s natural inflammatory response, most noticeable in the first 48 hours and often peaking around day two
Bruising – small purple or blue marks at or near the injection points; the extent varies from person to person
Redness – usually fades within a few hours
Tenderness – mild soreness when touching or chewing
Lumps or bumps – small firm areas as the filler integrates with your tissue; these typically resolve within one to two weeks, and you should never massage them unless instructed
Slight asymmetry – one side may swell more than the other early on
None of these mean something went wrong. They’re a normal response to a needle or cannula passing through the skin. Lips tend to swell more than any other area because the tissue is thin and richly supplied with blood vessels.
The first 24 hours matter most. The goals are simple: reduce inflammation and keep pressure off the treated area while the filler settles into place.
Minimize touching or pressing the treated area. Avoid rubbing, massaging, or “testing” the filler with your fingers. Skip facials and firm face washing. With lip filler, try not to purse your lips around straws or narrow bottles.
Apply cold compresses as needed. Use a clean, cool compress or an ice pack wrapped in a thin cloth for 10–15 minutes at a time, several times a day. Cold constricts blood vessels, limiting swelling and bruising. Never apply ice directly to skin, and don’t press hard.
Stay hydrated. Hyaluronic acid binds water. Drinking throughout the day supports smoother, plumper results and helps your body process mild inflammation.
Sleep elevated. An extra pillow keeps fluid from pooling overnight. Sleep on your back – not face-down or on the treated side.
You can eat and drink normally, though very hot liquids may feel uncomfortable if your lips are still numb from local anesthetic.
Monitor for peak swelling and bruising. This is when things usually look their most dramatic. Lips can appear noticeably larger than the final result, and bruises often deepen in color before they fade. Don’t judge your outcome now – it’s the least accurate view you’ll get.
Continue gentle care and hydration. Keep drinking water and using cold compresses intermittently. Reducing salty foods for a couple of days can limit fluid retention. Arnica gel or a bruise-friendly topical may help if your provider approves it.
Keep sleeping elevated and continue avoiding pressure on the area.
Hold off on workouts. Elevated heart rate and blood pressure increase swelling and worsen bruising. A gentle walk is fine.
If you have an event coming up, this is why we recommend booking filler at least two weeks in advance.
Observe reduction in side effects. Puffiness should be visibly going down, and bruises typically shift from purple or blue to green-yellow as they heal. Tenderness usually resolves around this point.
Maintain gentle cleansing and avoid irritation. Wash with a mild cleanser and lukewarm water using light, circular motions. Hold off on retinoids, strong acids, exfoliating scrubs, and treatments like microneedling until your provider gives the okay.
Resume light activity. Walking and gentle movement are generally fine at this stage.
Concealer is fine if you’d like to cover remaining bruising – apply with a clean brush or fingertips.
Resume your normal skincare routine. Most people can return to regular products, including actives, around the one-week mark. Reintroduce retinoids and acids one at a time so you can tell how your skin responds.
Protect skin from sun and heat. Use a broad-spectrum SPF 30 or higher daily. UV exposure breaks down collagen and can prolong discoloration in healing bruises. Continue skipping saunas, steam rooms, and hot yoga until swelling is fully gone.
Enjoy the shape settling in. By day seven, filler has usually softened and integrated enough that you’re seeing something very close to your true result.
Most dermal filler results are fully settled at two weeks. Residual swelling has cleared, any earlier lumps should be gone, and the product has fully integrated with surrounding tissue. If you’re evaluating symmetry, volume, or whether you’d like a bit more product, two weeks is the right time to assess – not day three.
The longevity of results depends on the product, the area treated, and your metabolism. Lip filler often lasts six to twelve months, while cheek and chin filler can last longer because those areas move less.
Wear sunscreen daily. Sun protection is the single most effective habit for preserving both your filler and your skin quality – UV exposure degrades collagen and hyaluronic acid alike.
Stay consistently hydrated. Ongoing hydration supports the water-binding action of hyaluronic acid fillers.
Don’t smoke. Smoking accelerates collagen breakdown and can shorten how long results last.
Space out your other treatments. Wait until swelling has fully resolved before scheduling microneedling, chemical peels, or dermaplaning over the same area.
Book maintenance before you’re empty. Touching up before filler fully dissolves usually requires less product than starting over.
Strenuous exercise – Skip the gym, running, and heavy lifting for 24 to 48 hours. Increased blood flow makes swelling and bruising worse. Stick to light walking.
Excessive heat – Avoid saunas, hot tubs, steam rooms, tanning beds, and very hot showers for at least 48 hours. Heat causes vasodilation, which encourages swelling.
Direct sun exposure – Stay out of prolonged, unprotected sun and away from tanning beds while healing. Healing skin is more susceptible to UV damage.
Non-prescribed blood thinners and supplements – such as aspirin, ibuprofen, fish oil, and high-dose vitamin E are avoided for a few days before and after treatment – but never stop a prescribed medication on your own. Ask your doctor first.
Facial massage, microdermabrasion, and dental work – Postpone non-urgent procedures that put pressure on the treated area for about two weeks.
Flying immediately after treatment – Cabin pressure changes can slightly amplify swelling. Give it a day or two if possible.

Good aftercare is mostly about restraint. For the first 24 hours, leave the area alone, ice gently, hydrate, and sleep elevated. Skip exercise, heat, and alcohol for a day or two. Expect swelling to peak around day two, fade through the end of the first week, and final results to settle at two weeks. In the long term, sunscreen and hydration do the heavy lifting.
Applying these dermal filler aftercare tips consistently is the simplest way to protect your investment and enjoy the refreshed, natural-looking result you came in for.
